 WAIVER*

Please acknowledge Waiver   ASSUMPTION OF RISK, WAIVER & RELEASE OF LIABILITY FOR MINOR. For the Parent/Guardian on behalf of the Minor: I hereby grant permission for my child, to be a part of the Swimsmart Academy, Aurora Ontario and to participate in activities arranged by the Swim Instructor. In return for the allowing the Minor to participate in this swim program, I, on behalf of the Minor and for myself, hereby waive, release and discharge Swimsmart Academy instructors, owners, operators for any and all claims for damages for personal injury, disability, death or property of any kind which may accrue to Minor or myself as a result of his/ her participation in this recreation program. This waiver and release is expressly intended to discharge in advance Swimsmart Academy and against any and all liability arising out of or connected in any way with Minor’s participation in this swim program. This Assumption of Risk, Waiver and Release of Liability shall apply to Minor and myself, as well as any of our heirs, executors or administrators. I hereby certify that I am the parent or legal guardian of Minor and that I am acting in that capacity. I agree to stay with the child during their lessons and ensure the safe entry and exit from the lesson. Further, I acknowledge that I have read this document and understand its contents.
